Current, signed, 10-year passport.
A letter signed by the applicant, explaining the need for a second passport.
One recent 2×2” color photo with white background.
Provide confirmed tickets or itinerary.
$160.00 + $35.00 execution fee (to acceptance agent)
$190.00 (no execution fee required if eligible for renewal by mail)
